Guardian Angel Open Enrollment Is Happening Now!!

Open Enrollment Starts: Wednesday, October 18, 2023

Open Enrollment Ends: Wednesday, November 15, 2023

Linked below is Guardian Angel’s Open Enrollment Health Insurance Packet for Coverage Period December 1, 2023 thru November 30, 2024, only available to our full-time employees. 

A full time employee is someone who:  Works at least 30 hours per week for Guardian Angel Staffing Agency, Inc..

Important Note:  Any full time employee who picks up Guardian Angel’s offered Health Insurance and fails to maintain full time status, “30 or more hours a week”, will lose their health insurance benefits.

Waiting Period :The eligibility period is the first of the month following 60 days from date of hire or change of status.

WHAT YOU NEED TO KNOW TO ENROLL:

If you are a full time employee and would like to enroll in one of the offered benefits then you:  Must complete Guardian Angel’s Election Form as well as the Respective Insurance Carrier’s Enrollment FormClick blue hyperlinks below to open the documents.

The Election Form and the Respective Insurance Carrier’s Enrollment Form must be completed in its entirety and returned to Guardian Angel Staffing by End of Your Open Enrollment.

Important Note:  Once you have made your elections, you will not be able to change them until the next open enrollment period unless you have a valid Qualifying Event.

Qualifying Events:                                                                                                                         

Changes to your elections may not be made outside Open Enrollment unless you have a Qualifying Event

Qualifying Events include:  marriage, divorce, legal separation, birth or adoption of a child, change in child’s dependent status, death of spouse, child or other qualified dependent, commencement or termination of adoption proceedings, or change in spouse’s benefits. 

You must notify Human Resources within 30 days of the qualified life event.  Depending on the type of event, you may be asked to provide proof of the event.  If you do not contact Human Resources within 30 days of the qualified event, you will have to wait until the next annual enrollment period to make changes (unless you experience another qualified life event).
